We always jump into conclusion and make summary based on perception and judgement. Our decision is being influenced by our past memories, whatever we see, we read and others. We might only make a decision to address the surface symptom and not look into the root of problem.
As such, the problem persists and it might happen in future as no complete solution is taken.
Think about it, if you are swimming, you can’t see the river. You need to stand at the shore, then you can see the river, fully.

This is a good reminder on how can we see things clearly.
